GENERAL MEETING MINUTES FOR SATURDAY,SEPTEMBER 9, 2006.
Attendees: Andy Lego, Lorraine King, Luci Kuziw, Mary Krauss, Tom Raymond, and Mike Walsh.
REPORT - SECRETARY
Tom motioned to waive reading the minutes from the last meeting. Andy seconded the motion.
Lorraine reported that all the USPS mailings listed below were completed and mailed. Lorraine reported that all e-mail notifications were sent.
June Newsletter - General Meeting Notification - Texas Hold Em Tournament. The next newsletter will go out in November 2006 . It will include information about the December 1 Memorial Mass and the January Dinner Dance. USPS mailings will go to FOLA members residing in Illinois only.
Elmcrest Banquet Hall secured for January Dinner Dance. Dick Biondi secured for January Dinner Dance.
Tom requested that a postcard should be sent to all FOLA members residing in Illinois that received paper mailings requesting RSVPs by September 29 for the Texas Hold Em Tournament. Attendees can call Tom on the phone or email him. Include other Tournament information on the postcard. Ladies Are Welcome To Participate in the Tournament.
Tom advised that solicitation is needed for the Silent Auction and Raffle Prizes for the January Dinner Dance. This issue to be discussed at a Dinner Dance Committee Meeting.
REPORT - TREASURER
Luci distributed a FOLA Financial Statement to all present. Financial Statements will be supplied, Upon Request, to to those who make donations.
Luci advised that the Not-for-Profit Certification for Friends of OLA has been approved and reinstated from the domestic incorporation date of May 27, 1998. It has been recorded in Cook County which is the county of the Friends of OLA President. It will need to be refiled every May as long as the organization exists.
Lorraine asked if Luci or Tom had a final ticket price for the January Dinner Dance. Response was that it should be available by the October 11 Board Meeting.
REPORT - EVENT COORDINATOR
Plans for the Texas Hold Em Tournament are complete. Schorsch Hall is secured. Tom requested that the Board start pushing for players. He needs RSVPs and Entry Checks bu September 29.
Tom will hold a committee meeting for the December 1st Memorial Mass after the Poker Tournament.
Luci said she would contact Colonial Bus Service to see if they will supply transporation from St. Francis Borgia Church to Holy Family Church and the return trip to St. Francis Borgia Church.
Father Boland has to be notified that FOLA plans to have the December 1st Memorial Mass at Holy Family.
Luci advised Tom that we need to request volunteers to do the reading, read names of victims by room number, light candles, present the gifts, etc.
Luci advised we need 100 votive candles.
Tom suggested that we give the last two recipients of the Scholarship Award the grant again at the January Dinner Dance. The Board Officers had no objections if candidates were still in school. Board suggested Tom contact Dan Consolazio of the Scholarship Committee to discuss.
Luci to contact the brother of Michele McBride regarding a past scholarship gesture.
THE DECEMBER 50TH ANNIVERSARY MEMORIAL MASS
Tom recommended that there be six committee members and one board representative. The committee will report to the board officers. Tom,as Event Coordinator, will represent the board. An email will go out in January 2007 requesting volunteers for the six committee member position.
COOK BOOK PROJECT
No update available at this time.

Betti Wasek called meeting to order at 12:15 p.m.
There were no minutes from previous meeting to read.
ELECTION
Dan Consolazio took roll call of board candidates.

It was noted that no Vice Presidential candidate was in attendance so the office remains open.
Since there were no contested positions, the election was held by a show of hands. All candidates were confirmed by attending qualified membership.
PRESIDENT'S REPORT
Betti discussed her plans for 2006 & 2007. a) Developing a newsletter and questionnaire asking members for their expectations from FOLA. b) Explore the possibility of a small membership fee or dues to defray the cost of mailings. c) Revising current by-laws. d) Establish a budget for FOLA. e) Establish a dollar goal for fundraising projects. f) Begin process of developing a budget for the 50th Anniversary Mass. g) Form committees for activities and events. Betti asked for membership patience in this transitional period.
Discussions followed regarding: Number of General Meetings Annually - Minimum four (4) Number of Board Meetings Annually - Minimum four (4) Accuracy of Mailing List Survey content and mailing Membership dues Contacting Cardinal's office for possibility of Cardinal saying the 50th Anniversary Mass
TREASURER'S REPORT
Luci circulated a copy of the FOLA Financial Recap for period 07/05/2005 through 03/06/2006.
a) Expenses were reviewed b) Donations reviewed c) Announced the 2006 Scholarship was delivered to recepient d) It was noted that there was no separate accounting of money taken in from the Silent Auction, Raffle, and donations received the evening of the Dinner Dance.
EVENT REPORT
Tom indicated that he would continue with the following events:
a) Annual December 1st Mass b) Annual Dinner Dance c) Pizza Night d) Fire Museum Trip
He announced new plans for a Texas Hold'Em Tournament.
A sign up sheet was circulated for committee volunteers for each event.
OLD BUSINESS
1) Quilt frame still needed to display the "Angel's Remembered" quilt 2) Fire Museum opens the 2nd and 4th Saturday of the month. The museum is on the third floor. An elevator is available to the second for only. The third floor can only be accessed by stairs.
NEW BUSINESS

Those in attendance at this meeting were Connie Rose Straube, Tony Fanone, Betti Wasek and Luci Kuziw.
The group continued to finalize plans for the annual dinner dance on January 28th. A very special thanks to Connie for all the wonderful donations she was able to receive for both the Silent Auction and the Raffle.
Volunteers are still needed to help out during the dinner.
A General Meeting is scheduled for Saturday, February 25th. The purpose of this meeting is the election of officers. For more information about the election process, please read the posted By-Laws. Officers must consist of Survivors, Family Members of Survivors and Victims of the OLA Fire and anyone who has attended Our Lady of the Angels School. One must be in attendance at the meeting in order to vote.
Watch the Main Message Board for additional information on location and time for this meeting as well as who to contact if you are interested in running for positions on the Board.
A meeting is scheduled for January 21st (location TBD) to finalize any last minute details prior to the dinner. If you are interested in attending, please contact either Betti Wasek or Luci Kuziw.

A meeting was held on December 10th regarding the upcoming Dinner Dance. Those in attendance where Betti Wasek, Connie Rose Straube, Andy Lego and Luci Kuziw.
A mailing regarding the January 28th dinner dance will be sent to everyone on both our current FOLA Mailing List and the one that we received from Linda Maffiola. This mailing will be done before the Christmas holidays.
A deposit will be sent to Elmcrest on 12/13.
Volunteers are still needed at the dinner to manage the welcome tables and to help with the Silent Auction and Raffle. The following have volunteered so far: Mary Campiani, Sally Konley and Luci Kuziw. More are needed. You can contact either Luci or Betti if you are interested.
Also, additional donations for both the silent auction and raffle would be appreciated. Again, you can contact any of the officers if you have something to donate.
Fr. Boland from Holy Family Church will be invited as our guest to the dinner and will offer prayers.
The North Side Fire Brigade through Bill Kugelman has donated their hall for future meetings and social gatherings for FOLA. It is located at 6240 West Addison in Chicago. Thank you, Bill for your kindness.
The group also discussed the 50th Anniversary Mass in some detail. Planning will begin in early 2006. We want to make this a very special and memorable occassion. We will also be recruiting volunteers for that. If anyone is interested, please let us know now.
The next meeting will held on Saturday, January 7th at 12:00 Noon at Betti's house. If you are interested in attending, please contact Betti for directions.
The election of FOLA Board Members was also discussed. Election will take place in February. More detailed information will be available in January.

The first item for discussion was the Dec. 1, Memorial Mass. Possible readers were discussed, selection of scripture readings, transportation by bus from St. Francis Borgia to Holy Family Church. Secretary Karen Raymond was asked to post a message on OLAfire site regarding the mass and request for food pantry items. Treasurer Luci Kuciw will bring and display candles for lighting, and will mail an announcement regarding the mass. Linda Maffiola will discuss readings with Fr. Boland, design a program and finalize details with the bus company. The second item for business was the Jan. 28, dinner-dance. More volunteers are needed for the raffle and welcome table. Patti Neri is working on centerpieces. Connie Straube made silent auction bid sheets and is distributing donation letters. More silent auction/raffle items are needed. Karen will send press releases to local media, and Linda Maffiola will send flyers to churches in the city and suburbs for bulletins and bulletin boards. A meeting will be held in December to review progress made.

FOLA October Fundraiser for 2004

Board Meeting Minutes held on June 22, 2004 In Attendance: Betti Wasek, Bill O'Brien, Mike Consiglio, Vicki Tortorich, Bob Mujica, Luci Kuziw, and Dan Consolazio.
1. Garden Committee: Reported on the completion of Father Joes Memorial garden, details concerning the dedication and what work was done to complete the garden. Luci notified us that the stone that was used for the plaque was donated by Meno Stone, a thank you card was sent out. Plants were donated by various companies thank you cards were also sent out. Bill, discussed the building of the garden, the water system and lighting that can be added in the future. Vicki will take pictures and have Eric post to the web site.
2. Night at the Races: A tentative date was set for Friday October 15, a mailing will be sent out in the middle of August with details.
3. Financial Report: Will be made available at the next general meeting to take place in September. After seeking legal council it was decided that we can not post records on the web site.
4. Dinner Banquet: Bob Mujica is working on plans for the dinner which is to take place after the first of the year.
5. Mr. Raymonds Scholarship: Dan and the appropriation committee along with John Raymond are working on who to award the funds to. Once decided the award information will be posted.
6. Betti suggested that we appoint an Advisory Committee, the names that were nominated were: Jim Grosso, Luci Kuziw, Dan Consolazio, and Linda Moffiola. The Board voted unanimously. All nominees have accepted.
7. Mailing List: an updated list is being worked on by Vicki, so far 80 names have been added from the website. A mailing will be sent out in August with a enclosed post card to all names on the original mailing list asking for updated information.

FOLA Board Meeting March 31 , 2004

Father Joe Memorial Garden Committee Members reviewed the bids obtained for Father Joe’s Memorial Garden to be located at Our Lady Mother of the Church at 8701 W. Lawrence Ave in Chicago. Morgan Landscaping received the job. We will meet with Father Dan. Monday April 5th to finalize the work and work should begin within the next couple of weeks.
We reviewed the Appropriations Committee Meeting which was held on March 24th . Members : Dan Consolazio, Jane Cozzi, Anthony Fannone, Matt Plavonich, and Betti Wasek Highlights from their meeting: Allocating funds for the Memorial Garden Scholarship to be awarded to child of a Firefighter to be given in the name of Mr. Raymond. Future Scholarships and to whom they will be awarded. Fundraiser and activities for 2004-2005 : Information will be posted soon.
Vicki Tortorich and Eric (Webmaster) will be working on a form to update mailing and email addresses in order to best send out Newsletters for future mailing. Form should be posted to the website shortly. You will also receive an email linking you to the form.

Minutes From the FOLA Meeting on February 26, 2004
Betti Marino-Wasek Opened Meeting: Introduced Board Members and Welcomed all in attendance
General Announcements:
Updated on the results of the Fund Raiser Dinner (January 10, 2004) We made a approximately $3000.00. 170 people attended the function.
Update on the Quilt, we have had a donation of the OLA emblem from Dolores Labuda. to be included on the quilt. The emblem will be placed next to Dolores sister Carol Gazzola name. Lorraine King has also donated an emblem. We wish to thank them both. The Quilt will be hung at Holy Family Church.
The By-Laws were voted on and will be posted on the web site on the Related Tab under Friends of OLA link.
Plans are underway to meet with the Pastor of Our Lady Mother of the Church to build a Memorial Garden in the name of Father Joe. The committees Members are Bill O'Brien, Vicki Tortorich and Luci Kuziw
Dan Consolazio was appointed to head the Appropriation committee, the appropriation committee will be responsible to over see the appropriate use of funds used for various projects. They will report directly to the board.
Jim Grosso is working on getting the former OLA School to be named a Historical site. Jim will be filling out the application and will bring the form to the meeting. We would like to have a plaque made to be placed on the "new" school. We will be working on the wording and the size of the plaque.
Bob Mujica has been working on future fundraiser ideas, some of functions he has been thinking of are: Picnic, Candle Light Bowl, and a day at the dog races.
